1/2. What makes a hero is up to the person, as we all have different opinions and may find different faults and flaws in one person and find another person perfect. You also may think the faults and flaws the person has makes them a hero, instead of making them a villain. As we all have different opinions, on one hand people may think for example... Robin Hood is a hero because he steals from the rich to give to the poor, but on the other hand, others may think his deeds are cheeky and villainous.
3. Heroes and Outlaws do not create themselves, but are created from our own thoughts and opinions because may view one person as a hero because of this and that whereas someone else may view that person as an outlaw because of this and that. Although, when a "hero" does good deeds it influences good opinions of that person therefore it is hard to find flaws and "evil" in that person making that person seen as a hero for the majority.
For questions one and two, I wrote it in an essay style. Separate the information I have given you and write them out in different answers.
